If you edit the icon image for a current stencil set item, and then edit the
Stencil Set Item, redirecting the Node Image path expression back to that
changed icon, you get the new icon in the Manage Stencils display of stencil
items, and (more importantly) in the Stencil working area itself, but when you
drag-n-drop that stencil the new Reference node appears with the old (unedited)
icon image.

What's more, if you go back into Manage Stencils and edit that stencil, the
icon displayed for the stencil item has gone back to the unedited version.

I tried to follow your instructions and everything worked fine for me.
So I can't be doing it right.

I opened the De Bono stencils set palette in Compendium. 

I then opened my Windows File Manager and navigated to the Stencil set folder
for the De Bono stenil set and found the nodeimage icon for the the black hat. 

I then used paintshop pro and edited the hat image and changed it's colour to
dark blue and saved it (with the same name as the black hat). 

I then went back to Compendium, editied the black hat stencils item
(double-clicked it on the palette) and pressed 'Save' on the item dialogt(as
the path had not changed, just the image iteself). The Stencil Dialog list and
the palette image updated corrently. I then saved the Stencils set and closed
the Stencsil Manager dialog.

I dragged the now dark blue hat palette item onto a map and the icon image
correctly showed the new dark blue hat icon, not the original black one.

What did I do differently to you?
